Steroids in asthma attack

Can we use hydrocortisone?

I haven't seen any evidence in textbooks but still it is commonly practiced in ED

IV hydrocort, IV Methylpred or oral prednisolone have almost equal efficacy.

Hydrocort more acute in onset, less anti-inflammatory but has significant mineralocorticoid effect too.

While other counterparts have more of anti-inflammatory property and less mineralocorticoid effect.

Oral if patient can tolerate is prefered over iv choices whenever possible.

Dexamethasone iv preferred when less chances of compliance to 3-5 days course post acute treatment as it is long acting.

As a junior doctor working in a resource-limited setting, I find your posts extremely helpful and practical. I have read all the posts and it has helped me understand the concepts better, take decisions and treat patients more confidently in the ED.
