Excellent work! Cardiac troponin is an exceptionally sensitive biomarker of myocardial injury, but an elevated troponin level does not, by itself, establish the diagnosis of a myocardial infarction. As a physician-scientist, I think it’s also very important for us to remind ourselves that troponin reflects cardiac muscle injury from a variety of causes. In addition to acute coronary syndromes, elevations may occur in conditions such as myocarditis, heart failure, pulmonary embolism, severe hypertension, sepsis, chronic kidney disease, strenuous exercise, and other forms of myocardial stress. Diagnosing a myocardial infarction requires integrating troponin trends with the patient’s symptoms, electrocardiogram findings, clinical history, and, when appropriate, imaging studies. Perhaps one of the most valuable lessons in clinical medicine is that biomarkers are powerful tools, but they are not diagnoses. The best patient care comes from combining laboratory data with careful clinical reasoning, ensuring that test results are interpreted in the context of the whole patient rather than in isolation.
